We are excited that you have decided to have your child baptized at St. Ursula Parish. In order to help you prepare for this special event in your family, there are several forms that need to be filled out. These are found below. We ask that you please call the parish office (410-661-0600) in order to receive a few additional items that we will send you, and to schedule yourself for the preparation class and the actual Baptism.

Forms needed for Baptisms (please bring as many of these to the preparation class):

• Parish registration form, if you are not registered and wish to register at St. Ursula. This is a PDF format. Please print it out and complete.
  • If you are registered, you need not fill out this form again.
  • If you are not a member of St. Ursula Parish, but are a member elsewhere, a letter of permission is needed from your parish. Bring this letter to the Baptism Preparation Class.

• The Baptismal Information Form. This is in PDF format. You can simply place your cursor in each field on the form, click radio buttons and type directly onto the page. Once you have it filled out the way you want, then print it. Bring your print-out to the Preparation Class.

• Eligibility statements for the Godparents and/or Christian Witness. A minimum of one Godparent is required. Please read the instructions at the top of these forms to determine which form best suits your needs. These are in PDF format. You can simply place your cursor in each field on the form, click radio buttons and type directly onto the page. Once you have it filled out the way you want, then print it and bring it to the Preparation Class.
  • A Godparent Eligibility form is required for Confirmed Catholics, of at least 16 years of age.
  • A Christian Witness Eligbility form is required for other non-Catholic, but Baptised Christians.
